Intuitively, continual immunisation of everyone against influenza continuously throughout their lifetime would be desirable, and may even lead to a better community outcome than predicted based on the 30-70% efficacy of the vaccine through the establishment of herd immunity. However, this scenario may, at times, render the community particularly vulnerable to influenza, especially if we confine ourselves to the vaccines that are typically in use today. Vaccines that exert their effects solely through induction of highly specific neutralising antibodies are beneficial, but their universal use may leave a population more susceptible in a scenario of vaccine mismatch or if a new subtype of virus emerges. Cross-protective responses, such as those invoked and continually boosted by natural infection, are probably the reason why most individuals experience severe influenza on only a few occasions during their lifetime.Similar concerns to these have been voiced by Rim-melzwaan's group in the context of the introduction of widespread vaccination of children. Vaccination of healthy children from 6 months of age against seasonal influenza has now been recommended in several countries including the USA and in Europe. Children are particularly at risk from seasonal influenza and also form a considerable reservoir of the virus, allowing spread to other sectors of the community, such as the vulnerable elderly. Clearly vaccination of this target group with antigenically well-matched vaccines has significant impact on disease burden.
