I read with great interest the description of a mini incision technique of dynamic hip screw (DHS) insertion by Waters et al. Certainly, minimally invasive surgery and mini incisions are very popular, and appear to benefit the patient.Having attempted the technique, I would make one suggestion. Through the small incision, triple reaming carries the risk of soft tissue damage. There does not appear to be a commercially made soft tissue protector with the standard DHS sets. A 50 ml syringe can be used, with the plunger removed and the end cut off at around the 30 ml mark (depending on the depth of the incision), to act as a soft tissue protector. It is placed over the guidewire and directed down on to the femur. The reamer is then inserted as usual, and is contained within the syringe. A similar technique has previously been described for reaming during intramedullary femoral nail insertion.
