Avascular necrosis of the femoral head is a recognised complication of traumatic posterior dislocation of the hip. However, this case is remarkable in two respects. Firstly, the injury sustained was a type 1 posterior dislocation8 which underwent reduction within 3 h. Secondly, there was an 8 year time delay between the injury and the first symptoms of avascular necrosis with no radiological evidence of AVN after 3 years. This has implications for both the Length of follow-up which may be considered appropriate following hip dislocation and the timing of medico-legal reporting.
