A 71-year-old man presented with a 3-day history of fever, rigors and progressive swelling, redness and blistering of both lower limbs, 5 days after wading knee deep in a local lagoon. Past medical history included ischaemic heart disease, type 2 diabetes, primary sclerosing cholan-gitis and long-standing lower limb swelling related to chronic venous stasis.On admission he was in septic shock with multi-system organ failure, including acute renal failure, disseminated intravascular coagulation and a severe lactic acidosis. There was evidence of severe lower limb cellulitis extending to the knees bilaterally, with prominent haemor-rhagic bullae (Fig. 1).
