Endean et al conducted a recent systematic review showing that occurrence of low back pain (LBP) is linked with abnormality of the spine on magnetic resonance imaging (MRI) with meta-estimates of odds ratios (OR) ranging from 2.3 (nerve root displacement or compression) to 3.6 (disc protrusion). However patients with LBP do not always exhibit any demonstrable pathology on MRI and spinal abnormalities can be found on asymptomatic patients. Most LBP epidemiological studies have pointed fairly consistently a causal role of physical risk factors with, in addition, psychological mechanisms playing an important role in the development and persistence of symptoms.
