Al-Habi and El-Shawarby should be commended for providing additional prospective data on the concentration of C-reactive protein (CRP) in the peri-operative period in patients undergoing elective neurosurgical procedures. However, while CRP does play an important role in the diagnosis of postoperative infections in selected cases, we do not believe that their own data supports their conclusion that CRP should be assessed in all patients undergoing standard neurosurgical procedures pre-opera-tively, post-operatively in the recovery room and during the patients' hospital stay.
