Objective: To study higher brain function with aging, cognition was studied qualitatively with the reaction time (RT) for right-left discrimination.Design: The cognition in right-left orientation was studied in a decision making task with RT.Materials and Methods: One hundred and eight volunteers, aged from 21 to 90 years, participated in this study (55 males and 53 females). They had no history of stroke, visual and auditory disorders. They got scores above 25 points with a questionnaire of the revised version of Hasegawa's Dementia Scale, and they showed no hemiparesis. They were asked to push a button quickly as they could at the same side as the cue presented in right-left discrimination, the cue was presented in 3 ways; 1) a sentence displaying on a TV-monitor, i. e., "Push the right button" or "Push the left button", 2) a white circle displaying on the monitor of black background at the right or left visual field, 3) click sounds (25, 40 or 60 decibel, Sound Pressure Level) generating from a headphone at the right or left ear.Results: The mean cognitive reaction time was 10.4 sec ± 4.42 for the sentence, 0.5 sec + 0.20 for the visual cues, and 0.6 sec + 0.47 for the auditory cues, respectively. The regression analysis showed the predictive equations; [RT]= 0.151 x [AGE] + 0.756 with the multiple coefficient of correlation of 0.25 for the sentence cue, [RT] = 0.005 x [AGE] + 0.115 with the multiple coefficient of correlation of 0.175 for the visual cues, and [RT] = 0.013 x [AGE] + 0.337 with the multiple coefficient of correlation of 0.189 for the auditory cues, respectively (Piason's test: P < 0.05).Conclusion: Right-left discrimination declined with aging. The cognitive responses to sentences had taken the longest time in right-left discrimination than those with visual or auditory cues. Right-left discrimination deteriorated with aging at the quickest pace for sentences than that for visual or auditory cues. Age-related decline for right-left orientation might be different from the right-left disorientation in Gerstmann's syndrome.
