We report a patient with an episode of hypothermia after cesarean section with combined spinal-epidural anesthesia (CSE) and postoperative epidural analgesia with morphine and levobupivacaine.A 41-year-old woman in her second pregnancy at term was scheduled for elective repeat cesarean section. Her previous cesarean section had been performed for failed engagement 10 years earlier under general anesthesia. CSE was administered at the L3/4 interspace in the sitting position with a 27-gauge Whitacre spinal needle through an 18-gauge Tuohy epidural needle and loss of resistance to air, without difficulty. Subarachnoid hyperbaric 0.5% bupivacaine 5 mg with sufentanil 5 mug and epidural 2% lidocaine 9 mL with epinephrine 1:200000 and sodium bicarbonate 1 mEq/10 mL were administered incrementally for surgical intervention Analgesic pin-prick level was T4 and loss of cold sensation was two segments higher.
