In this, the second article in a two-part series, AOIFE BATT discusses cleaning, disinfection and sterilisation of flexible endoscopes. There is an exponential relationship between the number of organisms killed and the time taken to kill them. This is one reason for the scrupulous manual cleaning of instruments and equipment prior to disinfection or sterilisation. An endoscope should be cleaned thoroughly, both internally and externally, between patients. Immediately after use, wipe the insertion tube surface with moist swabs soaked in a suitable disinfectant and flush the biopsy channel using the air and water button. This will clear material from the distal portion of the scope. Most modern endoscopes can be fully immersed once the endoscope hasbeen disconnected from the camera and light source equipment and a water-resistant plug/cap has been attached. This plug/cap is also suitable for ethylene oxide (ETO) sterilisation. Before immersing the scope, the leak tester should be attached to the valve located at the light guide connector (see images from previous article in this series: July 2004, pp431-432). By squeezing the bulb on the leak tester, air will be pumped into the scope and the pressure will be indicated on the dial If the pressure remains constant it is safe to immerse the endoscope (see Figure 1). Once it has been immersed, examine all surfaces, paying particular attention to the bending section at the distal end of the endoscope. A leak here may be visible only when the directional controls are used to bend the section in all directions under water. If the needle of the leak tester shows steady decreasing pressure, the scope should be removed immediately, dried, packed appropriately and returned to the manufacturer for repair. Not all leaks are obvious and may go undetected until moisture contaminates the optical system, which is evident by 'fogging' of the visual field.
