Sir, The lichenoid tissue reaction (LTR) is the most common form of a T-cell-mediated disease state. Many clinically heterogeneous skin diseases, ranging from lupus erythematosus to fixed drug eruption, have various elements of LTR. Histologically, it is characterized by dense interface lymphocyte infiltration and the formation of colloidal bodies. These ovoid eosinophilic bodies are often surrounded by 'satellite' lymphocytes. The presence of dyskeratotickeratinocytes and satellite-cell necrosis are common features of LTR. This apoptotic process is mediated by CD8+ cytotoxic T lymphocytes (CTCL) and natural killer (NK) cells. The two main distinct pathways, the perform/granzymeB pathway and the CD-95/CD-95L pathway, arethought to be involved in this apoptotic process in the LTR. Thus, apoptosis is thought to be the mechanism of cell death in the LTR.Apoptosis is a physiologically important process leading to a highly regulated, programmed form of cell death. Programmed cell death is a normal part of growth and development and oncogenesis. This process is controlled by numerous proteins, including a family of cysteine aspaxtic acid proteases known as caspases. Caspases regulate apoptosis by cleaving target proteins at specific amnio-acid residues at several steps within a proteolytic cascade.
