Lower Extremity Vascular Assessment in Patients with Diabetic Foot Ulceration

Diabetic foot ulceration is the leading cause of amputation in the developed world. Multiple recognized factors lead to amputation in this setting and the presence of peripheral arterial disease (PAD) is one of the most pivotal. Early recognition and intervention can often facilitate and expedite limb salvage efforts. More importantly, patients with PAD usually have a systemic atherosclerotic process that affects multiple organ systems and is associated with increased morbidity and mortality in diabetics. Early identification of systemic vascular disease and aggressive medical management with control of risk factors impact overall morbidity and mortality in the diabetic population. In this article, we explore the current technologies available to assess for lower extremity vascular disease. Though methods of non-invasive assessment are emphasized, the role of interventional methods, such as direct arteriography, is also discussed.
