Methotrexate is an antifolate agent used at high dose (i.e., > 1 g/m2 intravenously) in the treatment of some tumors (acute lymphoblastic leukemia, non-Hodgkin lymphomas, osteosar-comas). Antibacterial agents, proton pump inhibitors, and non-steroidal anti-inflammatory agents are known to affect methotrexate elimination [ 1 ]. We report a case of unexpected and life-threatening toxicity following administration of high-dose methotrexate in a patient receiving ciprofioxacin.
