Stage I seminoma is highly curable, but a tumor size of >4cm and invasion of the rete testis are risk factors for relapse. Conventional treatment with postorchiectomy irradiation has come under scrutiny because of concerns over the risk of secondary malignancies and late cardiovascular morbidity. Alternative postorchiectomy management strategies are surveillance and adjuvant chemotherapy.
