Approximately 35% of patients undergoing a radical prostatectomy for prostate cancer will experience biochemical recurrence within 10 years of surgery. High Gleason score, extracapsular extension, seminal vesicle invasion, and positive surgical margins have all been identified as risk factors for post-prostatectomy recurrence. Several studies have shown that patients with unfavourable prognostic factors are at increased risk of local and distant failure after prostatectomy, suggesting that these patients would derive particular benefit from postoperative radiotherapy. The rationale for adjuvant radiotherapy is based on several factors.
