Functional MRI techniques are increasingly being used for prostate cancer imaging, but their implementation differs markedly among institutions. A set of recommendations has now been published in an attempt to standardize protocols; however, the guidelines were devised without the use of evidence-based data, and require further scrutiny.The incidence of prostate cancer is rising as a result of increased PSA testing and an aging population. Although the cancer is often indolent, the underlying biology can be highly variable, which makes the decision of whether to treat-and determining the best time point at which to do so-difficult. Furthermore, as with other cancers, disease staging, monitoring and post-treatment detection of recurrence remain crucial determinants of disease management.
