In Parkinson's disease (PD) dementia occurs in up to 40% of patients. Its neuropathological substrate is still controversial. We report clinical and neuropathological findings in a patient with PD who developed dementia during the course of the disease. We show that in PD, Pick's pathology may also contribute to cognitive decline.The patient was a 62-year-old woman presenting with right hand resting tremor, with good levodopa response. Two years later, she suffered from depressive symptoms and episodes of disorientation.
