High-density lipoprotein (HDL) has proved to be a more complicated lipoprotein than was once believed. However, it is still clear that lower high-density-lipoprotein cholesterol (HDL-C) values are linked with increased cardiovascular (CV) risks, while higher HDL-C values in a population are associated with lower CV risks. There are data that indicate that raising HDL-C reduces future CV risk.1 The Coronary Drug Project showed that niacin-based therapy reduces mortality, while the Veterans Affairs High-Density Lipoprotein Intervention Trial (VA-HIT) and the Helsinki Heart Study demonstrated that use of fibrates reduces composite endpoints.
