摘要

Malignant pleural mesothelioma is an aggressive and fatal tumour of the pleura associated with previous asbestos exposure. Mesothelioma incidence varies from less than one case per million per annum in Tunisia and Morocco, to much higher rates in Australia, the United Kingdom and Belgium (approximately 30 patients per million per annum). Current therapeutic options include surgery, chemotherapy and radiotherapy or combinations thereof, all of which have poor results. Specifically, the role of surgery has been questioned after the publication of the Mesothelioma and Radical Surgery I trial and is now recommended only in clinical trials. With regard to chemotherapy, the most frequently used first line regimen is cisplatin combined with the antifolate pemetrexed. However, the median survival of patients who receive this regimen is only 9.3-13.3 months. Radiotherapy in mesothelioma is also limited by big target volumes and the presence of vital structures in the immediate neighbourhood (spinal cord, contralateral lung, heart and liver).
