We read with interest Chen et al.'s article on the feasibility of drug administration via the laryngeal mask airway (LMA) in a non-arrest adult porcine model.1 The authors found that adrenaline (epinephrine) delivered via a catheter through the LMA into the trachea can provide a similar effect to administration via a tracheal tube in terms of plasma adrenaline levels, heart rate elevation and mean arterial blood pressure change. However, these values were lower in the group receiving adrenaline at the upper end of the LMA suggesting that drug absorption was diminished when administered via this route. The reduced drug absorption by the laryngeal mucosa may be due to its smaller surface area relative to the larger broncho-alveolar area. In this study, insertion of the catheter into the trachea was guided by fibreoptic-bronchoscopy (FOB).
