A chain of events that leads to an incident seems like a good model to use in investigating accidents and injuries.But the logic behind the chain may be its weakest link.The chain is both a tool and a symbol that is familiar to one and all.Almost inseparable from any thinking involving a chain is the notion that a chain will fail at its weakest link.
