Some time ago, an American colleague asked me (RV) how I came to study uremic toxicity. My answer was, "sheer coincidence." I indeed entered this kind of undertaking simply by finalizing a half finished study about factors influencing protein binding of drugs in uremia (1). This work drew my attention to the role of protein bound uremic retention solutes, yet also to the complexity of metabolic disturbances in uremia, in analogy with the "uremic puzzle" displayed one day by one of my teachers, S Ringoir, at the Marrakesh Forum on Contemporary Management in Renal Failure in 1985.
