We greatly appreciate Dr. Wiwanikit' s valuable comments regarding the benefit of self-expandable metallic stent (SEMS) for malignant biliary obstruction. We would like to reply to his questions as follows. First, quality of life in patients with internal drainage has been shown by our study [1] to be much better than in patients with percutaneous biliary drainage (PTBD) regarding pain, since significant numbers of patients with PTBD required narcotic medications for pain control. We demonstrated that pain requiring oral narcotic developed in 71% (5/7) of our PTBD patients. In addition, metabolic derangement has been described as an important complication that can develop after external drainage [2]. Moreover, patients with plastic stent (PS) who lived long often suffered from PS clogging and cholangitis.
