A 76-year-old man post aortic valve replacement underwent a dual-chamber permanent pacemaker implantation due to sinus node dysfunction, intermittent high-degree atrioventricular block, and paroxysmal atrial fibrillation. At his last pacemaker check out, a concern was raised that there was malfunction of one of his pacemaker leads, even though the sensing, impedance, and threshold were within normal ranges. The patient approached our clinic for a second opinion, 6 months after implantation. A baseline 12-lead electrocardiogram (ECG) prior to pacemaker implantation is shown in Figure 1. Figures 2A and B show the 12-lead ECG after dual-chamber pacemaker implantation in two different occasions.
