Clinical Record of Emergency Vascular Access Using Adult Intraosseous (IO) Devices

Accessing the medullary space of the bone, offers a rapid, effective, and reasonably foolproof alternative to IV access. Newly introduced adult intraosseous (IO) technologies have been suggested as one means to improve success rate of emergency vascular access. The National Academy of Sciences Institute of Medicine Report recommends IO infusion as the preferred initial vascular route for combat casualty care. This review compares IO access devices, with a focus on the clinical record of the five FDA approved adult IO devices in use today and of one in advanced development. Published adult IO case reports, studies and clinical trials were identified and suggest utility and effectiveness as well as documenting complication rates. IO vascular access devices appear to fill a special need for combat casualty care, but independently funded cohort analyses and randomized controlled trials are needed to fully evaluate efficacy and safety.
