Recent evidence reports that many individuals in developing countries do not find the cash price for health services to be a barrier to their seeking care. When they are sick, they pay for health care, as necessary, and to a much greater degree than policy research had indicated before. Donor agencies and national governments, seeking solutions to the problem of recurrent cost financing, can make use of these findings. It is hypothesized that consumers are currently paying high fractions of their incomes for traditional and fringe-Western care (injectionists, 'black baggers,' and medicine vendors, among others), supporting a freely operating market for such care. It is hypothesized, further, that the level at which consumers are paying for such care is sufficient to support simple Western primary health care. Experience in Senegal and the Philippines demonstrates that primary health care can be produced at very low cost. Assuming that and that consumers are currently paying for care at a level which is within that cost of production, governments may be willing to help create the market for modern, self-pay, primary care. These are empirical issues. The recommendation of this report is to conduct sample surveys, to be used to design policies which expand coverage using existing, limited public and private resources.
