The preponderance of available information indicates that in some patients under controlled conditions, continuous subcutaneous insulin infusion (CSII) pumps are capable of reducing the blood glucose level of patients with insulin dependent diabetes mellitus (IDDM) to a near-normoglycemic state. Therapeutic trials of this device have been limited to carefully selected subjects afforded particularly close professional supervision. Available data do not demonstrate the superiority of CSII methods when compared with the results of equally intensive regimens using multiple manual insulin injections. The value of sustained normoglycemia in preventing diabetic complications also remains to be proven. The safety of insulin infusion pumps under routine conditions of widespread use among IDDM patients continues to be investigated. Questions remain concerning appropriate insulin preparations, therapeutic goals, and pump regimens. Until such information is available, CSII is considered to be investigational.
