摘要

Screening for latent tuberculosis infection (LTBI) is a mandatory attendance requirement at all healthcare professional schools. Currently in California, the Tuberculin skin test (TST) is the only approved test for LTBI in healthcare workers. In other states, the Interferon gamma release assay (IGRA) is slowly becoming an acceptable method in primary LTBI screening. The TST is fraught with issues including poor specificity, poor patient compliance, and a potentially expensive and prolonged anti-mycobacterial regimen if performed incorrectly. The authors retrospectively examined the records of TST and IGRA results from two medical student populations: one in California and one in Oregon, a state where the IGRA is an approved primary screening test. Based on data gathered from 2011 to 2014, the authors identified an LTBI prevalence of 15.8% using the TST method and a 13.5% using the IGRA method, which are consistent with published LTBI prevalence among healthcare workers. In the study, they also calculated the total cost of screening to a seronegative student over the course of 4 years and found that the IGRA along with a tuberculosis (TB) symptoms checklist is a less expensive and more convenient LTBI screening option for both the student and student health administrators. They authors recommend transition to IGRA with a TB symptoms checklist as the primary test of choice in the LTBI screening of medical students.
