The invention relates to a system (1) for the personalised monitoring of the health status of at least one patient (2) at risk of vital function failure or at risk of a recurrence or relapse of a disease, for the purpose of automatically and correctly generating suitable alarms intended for health professionals (3). The system of the invention comprises at least one means (4) for providing monitoring information to an information system (5), at least one item of personalisation data specific to each patient, an information system (5) executing a personalised monitoring program for each patient, and at least one means (8) for alerting at least one health professional and/or one assistant (3) when an alarm is generated. The invention further relates to a device and an associated method.
