FIELD: medicine.;SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics, and can be used for surgical treatment of comminuted thoracic and lumbar vertebral fractures. After the surgical approach to the spinal column in the trauma area, a transpedicular spinal fixation is performed with bilateral introduction of two screws into adjacent with at least one injured vertebra and one screw into fragment of at least one injured vertebra and their attachment to each other by means of corresponding rods and nuts. Screw introduced into the injured vertebra is installed on the side, which is contralateral acceptable for performance of anterior-lateral approach, providing its carrying through the pedicle of the arch of the injured vertebra into its largest fragment without leaving the boundaries of the lateral third of the injured vertebral body. In developing osteolysis of the latter, a second surgical intervention is performed, wherein through an anterior-lateral approach, a partial resection of the injured vertebra and a supporting corporodesis with an implant is performed without re-mounting the previously established transpedicular system. In case of trauma more than one of adjacent vertebrae one screw is installed in each injured vertebra.;EFFECT: method provides higher stability of fixation of the injured vertebra with eliminating obstacles for the anterior support corporodesis and reduced traumatism by excluding the stage of the transpedicular system remodeling in the cases of developing osteolysis after the first surgical intervention.;1 cl, 1 ex
