The present invention relates to a pharmaceutical composition in the form of powder for the treatment of moderate or advanced Alzheimer's disease-associated cognitive impairment, comprising: I) memantine or a pharmaceutically acceptable salt thereof in the form of prolonged release microgranules with a particle size less than 1000 microns comprising: - cores consisting of inert pellets with a particle size comprised between 500 and 600 microns, - a first hypromellose coating, - a second coating comprising memantine or a pharmaceutically acceptable salt thereof, povidone, and talc, - a third ethylcellulose coating, and - a fourth coating comprising hypromellose and talc; and II) an immediate release granular donepezil preparation with donepezil or a pharmaceutically acceptable salt thereof; a sweetening agent and a diluting agent selected from sugar, sorbitol, mannitol, and xylitol; with a particle size less than 710 microns.
