PROBLEM TO BE SOLVED: To understand and recognize at a glance the kinds and contents of care services that a person to be cared to receiver, scheduled time of execution of the services and further actual execution or non-execution thereof and others. ;SOLUTION: This table has a space 20 for showing a, day and a day of the week and a care execution schedule space 30 which shows the kind of care service in each section and is provided with a check area 33 for care services executed corresponding to each day entered in the space 20. The day of the week corresponding to the day in the month using the services, which is inscribed beforehand, can be entered in the space 20 arbitrarily, while the at-home care scheduled time, planned cares, check marks for completion thereof, etc., are entered in the care execution schedule space 30 in respect to each sort of care service. Care schedule tables for one month are constituted of a first schedule table main body 1 prepared by arranging the care execution schedule space 30 for one week in four sections in total as a care schedule table for four weeks and of a second schedule table main body 8 prepared by presenting the care execution schedule space 30 for three days by sections as a care schedule table for three days. Six sheets of the first schedule table main bodies 1 are disposed sequentially from the front and three sheets of the second schedule table main bodies 8 from the rear, so that they are composed in the form of a six-month calendar which is used by turning them over.;COPYRIGHT: (C)2001,JPO
