    • 摘要: OBJECTIVE To investigate the effects of Tongmai Yangxin Pill(TMYXP)combined with metoprolol tartrate or metoprolol alone for the treatment of premature ventricular complex(PVC)in patients with symptomatic frequent PVC.METHODS A total of 584 patients with symptomatic frequent PVC were randomly assigned(in a 1:1 ratio)into two groups:study group[n=292,TMYXP(40 pills twice/day,orally)combined with metoprolol tartrate(25 mg twice/day,orally)]and control group[n=292,metoprolol tartrate(25 mg twice/day,orally)plus placebo pill(40 pills twice/day,orally)].The total treatment period was eight weeks.RESULTS After eight weeks of treatment,the total effective rate of reduction of PVC in the study group and the control group were 76.4%and 51.4%,respectively(P<0.001).TMYXP combined with metoprolol tartrate demonstrated a significantly greater reduction of the frequency of PVCs compared with the metoprolol tartrate alone(−4537 times/24 h vs.−3013 times/24 h,P<0.001).The study group also showed a better result compared with the control group with respect to PVC related symptoms.In terms of New York Heart Association classification improvement,the total effective rates were 21.9%in the study group and 12.4%in the control group(P<0.05).Both the study group and the control group exhibited improvements in echocardiographic indexes.Left ventricular ejection fraction was significantly improved in the study group compared with the control group(P<0.05).There was no significant difference in the incidence of adverse events between the two groups.CONCLUSIONS Compared with metoprolol tartrate alone,TMYXP combined with metoprolol tartrate could more effectively reduce the frequency of PVC and alleviated PVC related symptoms,and improve cardiac function in patients with symptomatic PVC.

    • 摘要: Cystic lesions are very commonly encountered entities in the breast. Among these, Complex Cystic Breast Masses (CCBMs), which contain both anechoic and echogenic components, can result in a variety of imaging appearances. These include cystic lesions with thick walls and/or internal septa, intracystic nodular lesions, and solid-cystic masses of varying compositions. Ultrasound is the mainstay for evaluating cystic lesions, and thus recognizing the imaging features appropriately and suggesting suitable interventional procedures are included in their management. In this pictorial essay, we describe the wide range of ultrasound appearances of CCBMs with a number of clinically encountered examples from our institution. This article would enhance the understanding of readers in possible differentials to be included in their clinical practice and to suggest appropriate further intervention, when deemed necessary.

    • 摘要: Introduction: Complex post-traumatic facial disfigurements are caused by civilian or ballistic trauma. Their repercussions are functional, aesthetic and psychological. The initial emergency or secondary management of facial damage is difficult. The objective is to report, through the clinical observation of three patients, the results of the management of complex facial dislocations in a context of limited technical resources. Patients and methods: Three complex post-traumatic facial dislocations were managed between May 2006 and June 2017. Results: Primary treatment and secondary repair were established on a case-by-case basis. Local autoplasty was the indication of choice. Multidisciplinary management resulted in satisfactory functional and cosmetic outcomes. Two of the patients were lost to follow-up before the end of treatment. Discussion: The complete management of facial dislocations remains difficult and complex, in an extreme exercise situation. In an emergency, the priority is to save the life of the injured person. Several factors are involved in the therapeutic decision: the choice of the maxillofacial reconstruction technique, the availability of the technical platform, the financial capacity and the psychological state of the patient and his entourage. Sequential and multidisciplinary treatment gives satisfactory results in terms of morphology, function and aesthetics, despite the limited technical resources available. Psychological support is an essential complement for successful socio-professional and family reintegration.

    • 摘要: Dear editor,This study presents a method to estimate vector miss distance (VMD) for complex objects based on scattering center (SC) models. In the method, the dependence of the SCs on the aspect angle of the light of sight (LOS) is applied in VMD estimation. This method requires only the radar echoes in a short time frame when the flying object approaches to the closet point of approach (CPA).

    • 摘要: The title compound, FeCl4(C5N2H6)(C5N2H5) consists of two [(C5N2H6) (C5N2H5)]+ organic cations and [FeCl4]- anion. The geometry of the iron ion is tetrahedral, formed by four chlorine atoms. The complex was characterized by single crystal X-ray diffraction, Fourier Transform Infrared spectroscopy, thermal analysis and UV-Visible spectroscopy. Hirshfeld surface analysis was also used for understanding the intermolecular interactions in the crystal packing. Single-crystal X-ray diffraction analysis indicates that this complex crystallizes in the monoclinic system, P21/c space group with a = 7.598 (3) Å, b = 13.694 (4) Å, c = 17.105 (5) Å, β = 97.203 (6)° V = 1765.7 (10) Å3 and Z = 4. The [FeCl4]- anion and [(C5N2H6)(C5N2H5)]+ cations are linked through three-dimensional hydrogen-bonding network consisting of N-H...Cl and π-π interactions. Hirshfeld surface analysis and the related 2D fingerprint plots reveal that the complex is dominated by N-H...Cl contacts.

    • 摘要: LDLRAD3 is a recently defined attachment and entry receptor for Venezuelan equine encephalitis virus(VEEV)1,a New World alphavirus that causes severe neurological disease in humans.Here we present near-atomic-resolution cryo-electron microscopy reconstructions of VEEV virus-like particles alone and in a complex with the ectodomains of LDLRAD3.Domain 1 of LDLRAD3 is a low-density lipoprotein receptor type-A module that binds to VEEV by wedging into a cleft created by two adjacent E2-E1 heterodimers in one trimeric spike,and engages domains A and B of E2 and the fusion loop in E1.
