摘要:
目的对比联合拉力交锁髓内钉系统(inter tan)与股骨近端髓内钉(PFNA)治疗高龄股骨粗隆间骨折的疗效。方法选取泰和县泰和医院选取2017年1月—2020年9月收治的80例高龄股骨粗隆间骨折患者,根据随机数字表法分为对照组和观察组,每组各40例。对照组患者采用PFNA治疗,观察组患者采用Inter Tan治疗,对比2组围术期相关指标(手术时间、术中出血量、骨折愈合时间、住院时间),手术前及手术后4个月髋关节功能评分量表(Harris)、膝关节功能(HSS)评分,术后6个月的疗效及术后并发症发生情况。结果观察组患者术中出血量、手术时间高于对照组,骨折愈合时间、住院时间短于对照组(P0.05),术后4个月观察组Harris评分、HSS评分均高于对照组(P0.05)。术后4个月观察组患者总不良反应发生率为7.50%,低于对照组的20.00%,但差异不具有统计学意义(P>0.05)。结论PFNA治疗更微创,而Inter Tan治疗高龄股骨粗隆间骨折疗效更佳,更有利于患者髋关节功能恢复,且安全性更高。

目的:观察PFNA与Inter Tan在老年股骨粗隆间骨折中的疗效。方法:对行手术治疗的老年股骨粗隆间骨折患者按内固定方式进行回顾性分析,根据纳入和排除标准选取有完整随访资料的60例患者作为研究对象,应用Inter Tan髓内钉治疗的30例患者作为对照组,PFNA治疗的30例患者作为观察组,比较两组患者手术时间、术中出血量、术后定期复查X片观察骨折愈合情况及末次随访髋关节Harris功能评分。结果:观察组患者在手术时间和术中出血量方面均少于对照组,差异有统计学意义(P0.05)。结论:PFNA在老年股骨粗隆间骨折手术中相比Inter Tan髓内钉具有操作简便省时、出血少、骨折愈合满意的优势,值得临床推广。

目的:比较Inter Tan和股骨近端防旋髓内钉(PFNA-Ⅱ)闭合复位内固定治疗老年股骨粗隆间骨折的效果及关节功能远期效果.方法:选择2016年1月—2018年1月我院骨科收治的120例老年股骨粗隆间骨折患者,随机分为两组,每组60例,两组均实施闭合复位内固定术,A组应用Inter Tan,B组应用PFNA-Ⅱ,比较两组手术情况、术后骨折对位情况、骨折愈合时间、髋关节功能评分.结果:B组手术时间较A组短(P0.05).结论:在老年股骨粗隆间骨折患者闭合复位内固定术中应用Inter Tan、PFNA-Ⅱ均具有良好的临床疗效,可有效改善髋关节功能.

In this paper we will see the model of Universe according to Dynamic Universe Model of Cosmology by visualizing various processes that are happening in the Universe as per experimental evidences. For simplifying the matter here, we will see in part 1: about the Galaxy life cycle, where the birth and death of Galaxies discussed. Probably Universe gives guidance for the movement of Galaxies. We call this Part 1: Thinking and Reproducing Universe or Mindless Universe? (Galaxy life cycle). We see every day Sun, Stars, Galaxies etc., dissipating enormous energy in the form of radiation by the way of fusion of Hydrogen to helium. So after sometime all the Hydrogen is spent and Universe will die, is it not? … Dynamic Universe Model says that the energy in the form of electromagnetic radiation passing grazingly near any gravitating mass changes in frequency and finally will convert into neutrinos (mass). Hence Dynamic Universe Model proposes another process where energy will be converted back into matter and the cycle energy to mass to energy continues, sustaining the Universe to maintain this present status for ever in this form something like a Steady state model without any expansion. This we will see in Part 2: Energy - Mass - Energy Cycle. After converting energy into mass “how various elements are formed and where they are formed?” will be next logical question. Dynamic Universe Model says that these various particles change into higher massive particles or may get bombarded into stars or planets and various elements are formed. Here we bifurcate the formation of elements into 6 processes. They are for Elementary particles and elements generated in frequency changing process, By Cosmic rays, By Small stars, By Large Stars, By Super Novae and Manmade elements By Neutron Stars. This we will discuss in Part 3: Nucleosynthesis.

Aim: This study was performed to evaluate the movement of the proximal segment following different methods of ramus osteotomy that is one of the side effects of orthognathic surgery. Theoretically, with intraoral verticosagittal ramus osteotomy, it can minimize the movement of the proximal segment. The changes in the intergonal distance of mandible and the angle of the ramus flaring in two methods of osteotomy have been compared in this study. Materials and Methods: In this randomized clinical trial, included 60 patients (32 males) with mandibular prognathism and without any asymmetry were selected and divided into two groups (n = 30). One group underwent bilateral sagittal split ramus osteotomy technique to achieve mandibular setback and the other by the intraoral verticosagittal ramus osteotomy technique. Intergonial width and inner-ramal angle in the transverse plane were measured on radiographs preoperatively and 1 and 12 weeks postoperatively. Data were analyzed using covariance test with the significance level set at P 0.1). Conclusion: Considering our findings, there was no significant difference between two ramus osteotomy techniques regarding changes in mandibular width and inter-ramal flaring angle.
