Although the efficacy of acupuncture therapy is becoming more accepted, the mechanism of itsaction remains unclear. Studies of local and distant microvascular changes during acupuncture point stimulation may give clues into the initial underlying physiological change that triggers the cascade of events leading to a therapeutic effect. Microvascular networks are more abundant in the skin at acupuncture points than at non-acupuncture points. By recording pulsations of the microvasculature when an acupuncture point is stimulated, our studies showed that microvascular networks within a 2 mm diameter area dilate and constrict synchronously to a greater degree than normal. Simultaneously microvascular networks at acupuncture points along the same meridian on both sides of the body also synchronously pulse at an increased amplitude. With the synchronous expansion and contraction of the microvascular networks not only is more blood taken into the areas, but sympathetic visceral afferents on the blood vessels are stimulated and result in transmission along neural reflex pathways to internal organs and other regions of the body. The basic mechanism of acupuncture is the activation of the microvasculature which then initiates the other known and unknown physiologic changes which improve cellular environments and promote healing.
