This paper describes the design and the implementation of a medical learning system based on cases devoted to fourth year students. It helps them to structure their reasoning as far as diagnosis is concerned. The associated learning environment enables the student to experiment real situations under the control of the computing system. The formalisation of the medical activity enables to define an adapted learning environment : the case based simulation. This article presents a frame for describing the medical diagnosis approaches and the general architecture of case based simulation model.
