Who Comes First? Matters of Tobacco Intervention in Health, Ethics, and Accountability: A Comparative Analysis of China and the United States

Health and public policies intersect in matters of tobacco use intervention. The myriad problems stemming from tobacco in the United States, for example, beg considerations ethical choices and government service.1 Therefore, a theory of administrative accountability becomes important in our examination of key administrative decisions and actions on tobacco control that appear to have demanded ethical choices in determining appropriate administrative behavior. Such theory supports the principle that government, as the most effective advocate for measures which protect citizens, should promote ethical public policy, i.e. that which safeguards the public interest, including public health and regional economy. We found theoretical accountability clearly applicable to exploration of Chinese and American government tobacco control interventions that attempt to curtail rates of mortality and morbidity associated with tobacco use and exposure. This paper first provides the background in which tobacco use and its impacts in the U.S. and China can be discerned. That overview includes an examination of demographic and cultural dimensions of the tobacco problem, as well as the challenges the two countries face in curtailing tobacco use. Based on data from the World Health Organization (WHO), the Global Youth Tobacco Survey, the Global Adult Tobacco Survey, CDC reports on China and the U.S., and the Global Tobacco Epidemic, we cross-analyze China and U.S tobacco access, consumer use, tobacco marketing and government tobacco control efforts and policies. The author concludes with reflection of "lessons learned" that may be helpful in global efforts to impede the devastating results of tobacco use, as well as identify emerging trends and ways governments, globally, can safeguard the public's U. S. interest.
