East Baton Rouge Emergency Medical Service (EBR EMS) has implemented a community health program with the aim to reach out to members of the community and bring care to them before they need to dial 911. Community paramedics have been trained to assess patients' living environments, social networks, and health, as well as educate patients on utilizing the tools provided to them to take care of themselves. Paramedics use iPads and e-Bridge software in order to video conference with emergency department physicians, receiving feedback on the care and status of the patient. The student team assessed certain environmental factors on emergency calls in order to assist EBR EMS in creating effective telemedicine guidelines to be implemented across the community paramedic program. The student team created a community health program manual for paramedics to train and as a future reference tool. The team further utilized observational data collected on emergency calls and on scheduled visits by assessing scheduling options based on enrolled patient zip codes, and by analyzing historical call data obtained by EBR EMS. This information was to demonstrate potential future sustainability growth outcomes of the community health program.
