摘要

Hazard identification for system design shall be initiated at the earliest possible stage. The Preliminary Hazard List (PHL), shall be performed to identify the main generic hazards and accident scenarios that may be associated with the system being acquired or developed. This initial step is critical to the successful execution of a system safety effort as it provides focus on the need for special design attention to safety concerns, taking into considerations historical experience and new inputs from end-users. This paper shall share on how the Defence Science & Technology Agency (DSTA) has worked with the Singapore Armed Forces (SAF) and the defence industries to develop the PHLs for the System Safety effort for our weapon system acquisitions. In addition, the paper will also be sharing examples from the Republic of Singapore Air Force's (RSAF) Safety Information System (SIS), on how it has helped to systematically capture, track and broadcast safety information that could ultimately be used to distil and fine-tune PHLs for acquisition projects.
