Four patients suffering from CSF flow disturbance at the craniovertebral junction and related symptomatic syringomyelia are presented., whose syrinx has markedly diminished after endoscopic 3rd ventriculostomy (ETV). Two of the patients had an accompanying hydrocephalus and the other 2 cases presented with a narrow ventricular system.As foramen magnum crowding caused by basilar impression and/or Chiari malformation did not change after ETV performed for CSF flow diversion in any of the patients, the therapeutic success of ETV in diminishing syringomyelia could only be explained by decreasing pathological CSF-pulsation within the posterior fossa.
