Neurophysiological and Neuroimaging Study on Thalamic Pain

Neurophysiological and neuroimaging study were performed in order to ellucidate the neural mechanism of thalamic pain. Electrophysiological study using microelectrode during the course of stereotactic thalamotomy in 19 cases with thalamic pain revealed that characteristic changes occurred in the thalamic sensory nuclei; change of spontaneous neural activity, disordered topographic representation of sensory responses, and irregular burst discharges. Both PET and depth microrecording study were also performed in 5 cases with thalamic pain. PET study revealed spot-shaped increased zone of rCBF or rCMRglu in the cortical central area and at the sensory thalamus where electrophysiological study showed focal increase of electrical activity. Functional changes in the sensory thalamus and hyperactivity in the cerebral cortex adjacent to the central sulcus might be the important factors which relate for the genesis of thalamic pain.
