The BBB is a dynamic interface between the blood and the I central nervous system (CNS) that controls the rate of influx and efflux of biological substances associated with brain metabolic processes and neuronal function. The BBB is defined by the endothelial cell layer strengthened by tight junctions. Extracellular matrix (ECM) molecules constitute the basement membrane underlying the vasculature and play a critical role in maintaining the integrity of the BBB. Disruption of the BBB plays a central role in the pathophys-iology of ischemia and reperfusion injury resulting in increased vascular permeability leading to vasogenic edema, inflammatory cell influx, and neuronal cell death. This review will discuss the role of MMPs and inflammatory molecules as important mediators of BBB disruption and focus on three potential therapeutic interventions to protect the barrier following focal ischemia: matrix metalloproteinase (MMP) inhibitors, hypothermia, and anti-inflammatory agents.
