International Spinal Cord Injury Standards and Data Sets

Survival of spinal cord injury (SCI) with a reasonable quality of life has increasingly become an expected outcome worldwide. As a result there is an increasing need for standards, data, and measures pertaining to SCI. The International Standards for the Neurological Classification of SCI (ISNCSCI)[1] are used to document impairments of motor and sensory function after SCI. These standards are now in the sixth edition. The ISNCSCI have increasingly been used to document remaining spinal cord function in clinical trials related to SCI. Despite success with the current standards, assessment of the spinal cord components of autonomic function that affect a majority of organs is lacking. Therefore, standards have been developed by representatives from the American Spinal Injury Association (ASIA), and the International Spinal Cord Society (ISCoS) [2]. These standards have been for review at the websites of ISCoS (www.iscos.org.uk) and ASIA (www.asia-spinalinjury.org). The goal of these standards is to describe the impact of SCI on particular organ systems function. The standards allow clinicians and researchers to describe the effects of SCI on bowel, urinary bladder, sexual, cardiovascular, sudomotor and other autonomic functions. Because of anatomic relationships, the standards include a general anatomic diagnosis for the impact of SCI on urinary bladder, bowel and sexual function. A secondary classification describes the detailed impact of injury on each of these separate organ systems. A similar though separate process is used for the autonomic control of cardiovascular and sudomotor function, including thermoregula-tion.
