Donor specific anti-HLA antibodies may be mediators of graft rejection in solid organ transplantation but little is known of their role in hematopoietic stem cell transplantation (HCT). In an ongoing study, we are evaluating recipients of matched unrelated donor HCT (10/10 allele match) for pre and post anti-HLA Class I and/or Class II antibodies in order to determine if the presence of antibody has an impact on the outcome unrelated hematopoietic stem cell transplantation. A total of 139 patients were screened for HLA- specific antibodies using LABScreen Mix (LMAS, One Lambda) and the antibodies identified with the single antigen bead assays (Class I and Class II, One Lambda) with a cut-off of 1000 mean fluorescence units (MFI). The patient sera were screened 30 days prior to transplantation, and at 30, 60, and 90 days post transplantation out to a maximum of 258 days.
