Glaucoma is an optic neuropathy that continues to amass evidence of its multifac-torial nature. Glaucoma specialists should also consider vascular factors, fluctuation in physiological parameters and neurogenic theories of glaucomatous damage. The aim of this study was to evaluate fluctuations in IOP, blood pressure (BP), ocular perfusion pressure (OPP) and retrobular blood flow comparing the effects of dorzolamide and timolol add-on therapies in open-angle glaucoma patients. Results indicate both add-on therapies significantly lowered IOP compared to latanoprost baseline, with a larger reduction with timolol. Fluctuations of OPP were dependent upon BP but not IOP fluctuations. Lower evening diastolic BP was linked to increased vascular resistivity in the central retinal artery.
