This paper will discuss some examples from historical accidents where a delayed or incorrect pilot response has ended in an accident; discuss some of the possible factors that influence the variability of a pilot’s response; draw on some of the real world warning rates that have been observed and the need to reduce unwanted or false cockpit warnings; and present arguments for the need of an automated “last second” corrective action by the flying machine to prevent destruction of itself and its occupants.
