High profile structural failures such as roof collapses and dramatic earthquake damage make news - they can be seen on television, in print and on the internet. Such failures are often studied in detail with codes and/or practice modified and improved as a result. But what about the less newsworthy failures or near failures? Nearly every experienced engineer has witnessed firsthand a "near miss", problem or failure that had, or could have had, serious safety, financial and/or schedule implications. Often these failures become the subject of litigation, with relevant facts invisible to anyone but the parties directly involved. In other cases the issue is one that the responsible parties prefer not to disclose to protect reputations or egos. This paper is written from the collective wisdom and experience of the SEAOC membership. It focuses on case studies of failures or "near misses" to share lessons learned to a broad audience. Anonymous case studies will showcase a range of potential problems that, in hindsight, could have easily been avoided. For example, case studies will demonstrate how seemingly simple remodeling projects can carry with them serious consequences and how overlooking minor coordination issues can have disastrous structural impacts. This paper is intended to cultivate a culture of learning and to facilitate candid dialog, communicate important lessons from one generation of engineers to the next, and help avoid repeating mistakes of the past.
