目的：对比分析经乳晕径路与胸骨前径路腔镜甲状腺手术治疗甲状腺良性肿瘤的临床效果。方法选择住院并接受甲状腺手术治疗的甲状腺良性肿瘤患者140例。用随机数字表法分为乳晕径路组和胸骨前径路组，每组各70例，乳晕径路组患者行经乳晕径路腔镜甲状腺手术，胸骨前径路组行经胸骨前径路腔镜甲状腺手术。比较两组患者的术中、术后情况及美容满意度。结果胸骨前径路组患者的手术时间、皮下隧道建立时间和皮瓣游离面积均小于乳晕径路组，差异有统计学意义（P＜0．05），两组患者的术中出血量比较差异无统计学意义（P ＞0．05）。胸骨前径路组患者的 VAS 得分低于乳晕径路组，差异有统计学意义（P ＜0．05），两组患者的住院时间和术后并发症比较差异无统计学意义（P ＞0．05）。胸骨前径路组和乳晕径路组患者对美容效果的满意度比较差异无统计学意义（P ＞0．05）。结论经乳晕径路与胸骨前径路腔镜甲状腺手术均具有安全、美观、有效的优点，胸骨前径路手术能进一步缩短手术时间，减小对病人的创伤，值得在临床推广应用。%Objective To compare and analyze the clinical effects between endoscopic thyroidectomy via breast areola approach and that via anterior chest approach for benign thyroid tumor.Methods Totally 140 inpatients of benign thyroid tumor undergoing thyroid surgery were selected and randomized into breast areola approach group (n =70)and anterior chest approach group (n =70).Intraop-erative,postoperative conditions and cosmetic satisfaction were compared between the two groups.Results in the path before sternum groups of patients with Operative time,subcutaneous tunnel establishment time and free flap area were significantly less in anterior chest approach group than in breast areola approach group (P <0.05),while there was no significant difference in intraoperative blood loss between the two groups (P >0.05).VAS score was significantly lower in anterior chest approach group than in breast areola approach group (P <0.05),while there were no significant differences in hospital stay and postoperative complications between the two groups (P >0.05).And there was no difference in cosmetic satisfaction between the two groups (P >0.05).Conclusions Endoscopic thy-roidectomies via breast areola approach and anterior chest approach are both safe and effective.The thyroidectomy via anterior chest ap-proach can shorten the operative time,reduce trauma to the patient,therefore,is worthy of promotion in clinic.
