目的 探讨腹腔镜肝切除(LH)的手术指证和手术技巧.方法 回顾性分析2009年1月～2011年2月对20例施行完全腹腔镜肝切除术患者手术时间、术中失血量及术后并发症等.结果 该组19例手术在完全腹腔镜下完成,1例分离第二肝门时损伤肝右静脉而中转开腹.手术时间110～400 min,平均218min,术中出血量50～ 800mL,平均305 mL.术后住院7～17d,平均9.5 d.无围手术期死亡及严重并发症发生.结论 合理掌握手术指证和技巧,可安全进行腹腔镜下肝叶切除术.%[Objective] To explore the indication and the techniques of clinic application of laparoscopic hepatectomy (LH). [ Methods ] The clinical data of 20 cases receiving the procedure in our hospital from January November 2009 to February 2011 were retrospectively analyzed. [Results] Laparoseopic hepatectomy was performed successfully under laparoscope in 19 cases and converted to open operation in 1 because of intra-operative hemorrhage. In laparoscopic hepatectomy, the median operative time was 21S min (110-400 min), the median blood loss was 305 mL (50-800 mL). No perioperational death and critical postoperative surgical complications occurred. The median length of postoperative hospital stay was 9.5 d (7-17 d). [Conclusion] Laparoscopic hepatectomy is safe and feasible by using reasonable indication and the techniques.
