剪切波弹性成像在乳腺良恶性肿块鉴别诊断中的应用价值

         

摘要

Objective To evaluate the application value of different kinds of elastic modulus value computed by shear wave elastrography (SWE) technology in differentiating benign and malignant breast solid lesions.Methods SWE technology was performed in 156 cases of breast solid lesions to detect the elastic modulus value including the elastic modulus maximum (Emax), the average elastic modulus (Emean), the elastic modulus distribution of quantitative indicators (Esd), and the ratio between the Emax of breast solid lesion and the Emax of normal breast tissue (Eratio). The diagnosing boundary value of the four different data was calculated respectively.Results The elastic modulus value of breast solid lesions were statistically signiifcant, including Emax, Emean, Esd, and Eratio. As the ROC curve displayed, when the Youden index reached the maximum, the sensitivity of Emax, Emean, Esd, and Eratio were 82.2%, 74.4%, 83.3%, and 85.7%; the speciifcity of Emax, Emean, Esd, and Eratio were 96.8%, 90.4%, 93.4%, 95.1%.Conclusion The elastic modulus value of breast solid lesions is of great signiifcance in the evaluation and differential diagnosis of benign and malignant breast solid lesions. Contrasted with the other elastic modulus value, the Eratio and the Emax have greater diagnostic value.%目的:应用剪切波弹性成像技术测定乳腺肿块的各项杨氏模量值,评价其在乳腺良恶性肿块鉴别诊断中的应用价值。方法测定156个乳腺肿块的Emax值、Emean值、Esd值、Eratio值,并计算这4种数据的诊断临界值。结果乳腺良恶性肿块的Emax值、Emean值、Esd值及Eratio值均具有统计学差异;经受试者工作特征(ROC)曲线分析,Emax值、Emean值、Esd值、Eratio值取约登指数最大点为诊断临界值时,灵敏度分别为82.2%、74.4%、83.3%、85.7%,特异度分别为96.8%、90.4%、93.4%、95.1%。结论 Emax值、Emean值、Esd值、Eratio值对乳腺良恶性肿块的鉴别诊断具有重要意义,其中Eratio值及Emax值的诊断意义较大。
