目的 探讨消银汤配合外用药治疗前后银屑病患者综合免疫状态及红细胞多项指标的变化规律.方法 选取2010年3月～2012年3月于我院进行治疗的90例银屑病患者为研究对象,将其随机分为对照组(泽它洗剂组)和观察组(消银汤配合泽它洗剂组),每组各45例,后将两组患者治疗前和治疗后2、4周的血清免疫球蛋白及其补体、外周血T淋巴细胞亚群及红细胞CD35分子、CD59分子、红细胞平均血红蛋白浓度(MCHC)、红细胞平均体积(MCV)、红细胞免疫黏附促进因子(RFER)、免疫黏附抑制因子(RFIR)的水平进行比较.结果 观察组治疗后2、4周的免疫球蛋白A(IgA)、免疫球蛋白G(IgG)及补体C3、C4均低于对照组,免疫球蛋白M(IgM)高于对照组,CD3+、CD4+、CD4/CD8及自然杀伤细胞(NK)均高于对照组,CD8+低于对照组,而红细胞CD35分子、CD59分子、RFIR及MCV均低于对照组,RFER及MCHC均高于对照组,差异均有统计学意义(均P ＜ 0.05).结论 消银汤配合外用药可显著改善银屑病患者综合免疫状态及红细胞多项指标,对于改善患者的疾病状态发挥着积极的临床作用.%Objective To study the variation of comprehensive immune state and red cell indices of patients with psoriasis before and after the treatment of Xiaoyin Decoction combined with external medicine. Methods 90 patients with psoriasis in our hospital from March 2010 to March 2012 were selected as research object, and they were randomly divided into control group (Zetar Lotion group) and observation group (Xiaoyin Decoction combined with Zetar Lotion group), with 45 cases in each group, then the serum immunoglobulin, peripheral blood T lymphocyte subsets and erythrocyte CD35 molecule, erythrocyte CD59 molecule, mean corpuscular hemoglobin concentration (MCHC), erythrocyte mean corpuscular volume (MCV), RBC rosette forming enhancement rate (RFER), RBC rosette forming inhibitor rate (RFIR) of two groups before and after the treatment at second and fourth week were compared. Results The immunoglobulin A (IgA), immunoglobulin G (IgG) and complement C3, C4 of observation group after the treatment at second and fourth week were all lower than those of control group, immunoglobulin M (IgM) were higher than those of control group, CD3+, CD4+, CD4/CD8 and NK were all higher than those of control group, CD8+ was lower than that of control group, erythrocyte CD35 molecule, erythrocyte CD59 molecule, RFIR and MCV were all lower than those of control group, RFER and MCHC were all higher than those of control group, there were all significant differences (all P < 0.05). Conclusion Xiaoyin Decoction combined with external medicine can significantly improve the comprehensive immune state and red cell indices of patients with psoriasis, and it also plays an active clinical role in improving the disease state of patients.
